PCA Care Connections Welcomes Two New Board Members

PCA Care Connections is pleased to welcome two new members to its Board of Directors, effective August 2021. Shani Gilmore has joined the board as treasurer and Denise V. Stewart has joined as an officer. The updated list of board members can be found below.

PCA Care Connections Receives NCQA Accreditation

We are pleased to announce that PCA Care Connections (PCA CC) has received an accreditation through the National Committee for Quality Assurance (NCQA). PCA CC received a three-year accreditation on July 22, 2021, after successfully completing and submitting the survey requirements in May 2021 and an NCQA virtual site visit for file review in July 2021.

As always, PCA CC is committed to providing high quality service to our participants, which is why we decided to apply for NCQA accreditation. NCQA offers a specific accreditation in Case Management Long Term Supports and Services (CM-LTSS) to help organizations improve their operations and initiatives in targeted areas and to demonstrate their commitment to quality. The standards provide a framework for organizations to deliver effective, person-centered care that meets consumers’ needs and aligns with state requirements. 

There are seven CM-LTSS standards that are being addressed to fulfill NCQA’s accreditation requirements: Program Description; Assessment Process; Person-Centered Care Planning and Monitoring; Care Transitions; Measurement and Quality Improvement; Staffing, Training and Verification; and Rights and Responsibilities. Each standard required extensive review, planning and in some cases, changing or creating new policies and procedures to fulfill accreditation requirements.

Key benefits of NCQA accreditation include improved outcomes for our regional LTSS population, as well as alignment with Pennsylvania Health and Wellness (PHW), the managed care organization we currently partner with under Community Health Choices (CHC) to provide service coordination. PHW requires all of their service coordination entities to be accredited by NCQA.

We want to thank our staff for their commitment to learning and implementing NCQA standards and requirements, which allowed us to be successful in this endeavor.
