Mission statement
To provide high quality, person-centered and cost effective Service Coordination for older adults and persons with disabilities in partnerships with managed care organizations and integrated health systems and practices. Our goals are to promote quality of life, independence, dignity, and optimal health and well-being.

Our Staff Qualifications
- Every PCA Care Connections Service Coordinator has a Bachelor’s degree in Social work, psychology or related field and/or 3 years’ experience in a social service or health care setting.
- Every Service Coordinator Supervisor has a Master’s degree in Social Work or related field and at least 3 years’ experience in a social service or health care setting.
