PCA Care Connections is a NCQA accredited non-profit agency that currently provides two types of services: Service Coordinator for Community Health Choices (CHC) participants enrolled with Pennsylvania Health and Wellness and Nursing Home Transition Services for participants enrolled with Pennsylvania Health and Wellness or Keystone First. We currently serve participants throughout the Greater Philadelphia region, including Philadelphia, Delaware, Montgomery, Bucks and Chester counties.

Service Coordination

PCA Care Connections service coordinators help older adults and those with disabilities remain as independent as possible in their own homes and communities. Our trained staff provides personalized services for those who need long-term support. Our diverse service coordination staff speaks numerous languages, including Spanish and Russian along with various other languages.

Our service coordinators:

  • Conduct a comprehensive needs assessment that include the individual's goals and preferences
  • Develop and implement a custom care plan, arranging services such as personal care, home-delivered meals and transportation
  • Connect individuals with appropriate community resources.
  • Assist with annual financial recertification
  • Support individuals as they transition out of hospital and rehabilitation settings and back into the community
  • Assist with obtaining employment for those interested
  • Monitor progress through visits and phone calls

Nursing Home Transition

PCA Care Connections’ Nursing Home Transition program assists residents of long-term care facilities who are interested in living independently transition back to community-based living. The goal of the program is to work closely with residents and community agencies to coordinate services to support the participant in their transition back to the community.

Making current homes accessible through home modifications
Coaching for independent living


Who is Eligible?

Must be enrolled in Community Health Choices and contracted with Keystone First or Pennsylvania Health and Wellness
Must be a resident of long-term care facility
Must be interested in returning to community-based living in Philadelphia, Delaware, Bucks, Montgomery or Chester County

Participant Rights and Responsibilities

All PCA Care Connections (PCA CC) participants have the right to:

  • Receive understandable information
  • Access information about PCA CC by accessing the website at http://www.pcacareconnections.org.
  • Access information about Community HealthChoices (CHC) and Pennsylvania Health and Wellness (PHW) including all covered services and how to file a complaint in the PHW Participant Handbook issued by PHW or at https://www.pahealthwellness.com/members/ltss/member-resources/handbooks-forms.html
  • Disenroll from the program or terminate any service
  • Know you service coordinator (SC). Participants and can contact PCA CC at 215-282-6500 to request change in SC or to report any complaints regarding PCA CC staff. If a complaint is made to PCA CC regarding PCA CC staff, a Supervisor will explain the process and timeframes involved when the complaint is reported.
  • Be supported by PCA CC to collaborate on decisions with the SC
  • Be informed of all covered and uncovered services and have the SC discuss options with you
  • Have protected health information (PHI) kept private be told when your PHI is shared with others.
  • Be treated courteously and respectfully by all PCA CC staff

PCA CC participants are responsible for:

  • Following the mutually agreed-on person-centered service plan or notify the SC if they cannot follow the plan
  • Provide PCA CC and long-term supports and services (LTSS) providers with the information necessary to deliver services
  • Notify PCA CC and any other providers if you cannot follow your agreed upon service plan or of your decision to disenroll from the program
